Thank you for your interest in West Shore Achieve Visionary Education (W.A.V.E.)​ After School program at West Shore Middle School. Please fill-out this application form completely. Filling out this form does not guarantee enrollment in this program. Enrollment will be selected based on first come first serve and/or lottery system By signing this form you accept W.A.V.E. behavioral and attendance policies.

• Behavioral​ ​Policy:​ All West Shore school rules and expectations will apply.

• Attendance​ ​Policy​: The expectation is that the child attends all 4 days with no more than a minimum of 10 excused absences. Daily and session attendance will be maintained for safety reasons and to monitor state attendance requirements for this state funded grant. ​ ​
