High school sports permission and emergency authorization forms are now available on Powerschool. All athletic forms are completed online and will be submitted directly to the Athletic Director.
Parents: To begin the process, log into your Powerschool parent portal account and on the left navigation menu choose eCollect Forms, then Athletic Permission/Emergency Authorization.
